2. First Degree Date
3. Assembly Name
4. Council Name
5. Applicant's signature
6. Proposer's Signature
7. Faithful Navigator's signature
8. Faithful Comptroller's signature
9. Applicant's Information
10. Candidate fee is $70 (includes Initiation Fee and Exemplification Banquet)
